1. If you belong to a Managed Care Plan (HMO) which requires authorization to see a specialist, you are responsible for obtaining the initial and any subsequent authorizations required. Our staff will be happy to assist you with authorizations required for further treatment.

2. If you belong to a Managed Care Plan (HMO) or a PPO plan, all applicable fees are due at the time of treatment. Please refer to your co-payment schedule or for application fees.

3. We will accept assignment of claims for the insurance companies listed on the previous page. We will file these claims for you. If your plan is not listed, you will receive a claim to file with your insurance, with the exception of Champus. Even though we do not participate with Champus, we are obligated to file a claim on your behalf. Full payment will be expected at the time of service from all patients insured by companies not listed on the previous page, including Champus patients.
